Surrogacy has become an increasingly common option for people facing infertility or same-sex couples who want to build a family. Naturally, the focus often rests on the intended parents and the child.

However, the surrogate mother plays an essential and deeply personal role in this journey. Along with the physical demands of pregnancy, surrogate mothers often experience a range of psychological challenges that deserve more attention and understanding.

Emotional attachment and separation

Even when a surrogate understands that the child is not hers biologically or legally, the experience of carrying a baby for nine months can naturally create emotional bonds. Letting go of the child after birth may lead to grief, sadness or even depression. To cope with this challenge, some surrogates receive psychological counseling both before and after the birth.

Pressure from intended parents and society

Surrogates can also face pressure from:

  • The intended parents
  • Medical practitioners
  • Society in general 

Some may feel scrutinized for their choices or judged based on their motives, especially if financial compensation is involved. 

Additionally, there may be implicit expectations from intended parents regarding the surrogate’s lifestyle, diet or medical decisions. This can create feelings of being monitored or controlled, which in turn may affect the surrogate’s sense of autonomy and self-worth. 

Identity and self-esteem issues

Surrogacy can sometimes blur personal boundaries, especially when the surrogate is trying to balance her role as a mother to her own children with the temporary role she plays for the intended parents. Feelings of low self-esteem may emerge if the surrogate does not receive adequate emotional support or validation from family and friends. 

While surrogacy offers joy and hope to intended parents, it can present real psychological challenges for surrogate mothers. Recognizing these challenges and enlisting continuous emotional and legal support is essential in helping ensure that the surrogate’s mental health is protected throughout the process.
